Privacy Policy and Practices of Lipstone Insurance
Agency Inc.
In 1999 Congress recently passed the Gramm-Leach-Bliley (GLB) Act,
which deals in part with how financial institutions treat nonpublic
personal financial information. The Act is effective July 1, 2001.
Lipstone Insurance Agency Inc. has always been committed to maintaining
customer confidentiality. We appreciate this opportunity to clarify
our privacy practices for you as a result of this new law.
• As part of our insurance business, we obtain certain "nonpublic
personal financial information" about you, which for ease of
reading we will refer to as "information" in this notice.
This information includes information we receive from you on applications
or other forms, information about your transactions with us, our
affiliates or others, and information we receive from a consumer
reporting agency.
• We restrict access to the information to authorized individuals
who need to know this information to provide service and products
to you.
• We maintain physical, electronic, and procedural safeguards
that protect your information.
• We do not disclose this information about you or any former
customers to anyone, except as permitted by law.
• Employees share this information outside the company only
as authorized by you or for a specific business purpose.
• The law permits us to share this information with our affiliates,
including insurance companies and insurance service providers.
We may also share other types of information
with our affiliates, including insurance companies and insurance
service providers. This information may be financial or other personal
information such as employment history and it may not be directly
related to our transaction with you. Consistent with the Fair Credit
Reporting Act, our standard authorizations permit us to share this
information with our affiliates.
You do not need to call, or do anything as a result of this notice.
It is meant to inform you of how we safeguard your nonpublic personal
financial information. You may wish to file this notice with your
insurance papers.

While the nature of insurance requires that we gather personal financial
and / or health information about you, we know that this information
must be protected. The Lipstone Insurance Agency, Inc. privacy notice
applies to information gathered in connection with the products underwritten
by the companies as described above.
Information We Collect:
We get most information directly from you, such as information provided
to us on your application for insurance. We also collect information
about your transactions with us, such as insurance premiums and policy
numbers. We may obtain additional information from third parties.
Third parties may include companies, employers, consumer reporting
agencies, or health care providers. Information collected may relate
to your finances, employment, health, avocations, or other personal
characteristics, as well as publicly available information about you.
Information We Disclose:
We may disclose collected information to our affiliates and non-affiliated
service providers when necessary to carry out our normal business
activities. These activities may include evaluating requests for insurance
products, evaluating claims, marketing services, administering of
policies, and processing transactions at your request. Service providers
may include your company agents who service your policies, our affiliates,
or third parties that help us provide you with our products and services.
We may also disclose collected information to financial institutions
with which we have a joint marketing agreement. Affiliated companies
are within the Lipstone Insurance Agency Inc. family of companies.
Non-affiliated companies are outside of the Lipstone Insurance Agency
Inc. family of companies. They may include banks, other insurance
companies, service vendors and insurance agencies. We also may disclose
information as permitted or required by law. We do not disclose collected
information about our former customers to anyone except as permitted
by law.
Protection of Information:
Our employees are trained and required to maintain our privacy policies
and procedures. Employees who violate these policies and procedures
are subject to disciplinary action. Affiliates and third parties to
whom we disclose information are required to maintain adequate security
standards for the protection of collected information. In addition,
